Our Transitional Aged Youth (TAY) Clinic is for youth aged 16 to 24 who:
- have been diagnosed with a mental health disorder and are a high risk of harming themselves or others, or
- are at risk of developing impairments due to a mental health diagnosis.
We help TAY patients to:
- be involved in their own care
- learn new skills and coping strategies
- maintain healthy lifestyles
- return to school, work and/or daily life
- smoothly transition into adulthood
This clinic offers:
- virtual care via telephone or video call platforms (e.g. OTN, Zoom)
- medication education and management
- psychosocial assessment
- up to ten individual counselling sessions and/or group programming. Family members may also be invited to attend individual sessions
- support for transitions to adult mental health care, addictions services, educational and/or employment opportunities
- referral to community services
- flexible hours, including evening and weekend appointments
TAY services are offered for up to six months.
Referrals
To be eligible for the TAY clinic, you must be 16 to 24 years old, live in the Central West region and have at least one of the following:
- a major mental illness (e.g. mood or anxiety disorder, OCD, personality disorder)
- new and emerging mental health concerns
- suicidal threats and or ideation
- concurrent substance misuse
Youth can request a referral to this program from their family physician or psychiatrist.
