As a sign of our resolve to provide targeted care for older adults, our hospitals are proud members of Nurses Improving Care for Health System Elders (NICHE). This international nursing education and consultation program is a leader in geriatric care in health care organizations.

Acute care for the elderly (ACE) |
Our ACE team helps patients remain independent after a hospital stay. Using a patient and family-centred approach, our seniors' experts create comprehensive geriatric care plans to support each patient's mobility and function while they are in the hospital. All ACE team members have specialized training in supporting the needs of geriatric patients. The ACE team includes:

Nurse led outreach team (NLOT) |
The NLOT provides care to seniors residing in long-term care homes in the community. They provide evaluations and treatments to prevent unnecessary admissions to hospital, as well as reduce the length of hospital stay. |
Geriatric outreach team (GOT) |
The GOT specializes in caring for seniors in our community. This team provides in-home assessments to at-risk seniors for whom leaving home is physically, mentally or logistically difficult. Assessments focus on functional, psychological, cognitive and physical health. The GOT collaborates with geriatricians and the health care team at the Centre for Seniors and Rehabilitation at Peel Memorial to best support seniors. |
Centre for Seniors and Rehabilitation
Our Centre for Seniors and Rehabilitation offers a team of health care professionals:
- geriatricians (physicians specialized in the care of seniors)
- nurse practitioners
- registered nurses
- physiotherapists
- occupational therapists
- speech-language pathologists
- social workers
- registered dietitians
- pharmacists
- team coordinators and clerical staff

Memory clinic |
The memory clinic diagnoses and treats seniors with a wide range of cognitive disorders. These include mild cognitive impairment, Alzheimer's disease, Lewy body dementia, frontotemporal dementia, Parkinson-related cognitive dysfunction and stroke-related memory dysfunction.
