This Low Back Pain Rapid Access Clinic is for people 18 and older who currently have low back pain that has lasted for at least six weeks, but no longer than a year.
Your doctor or nurse practitioner must refer you to this clinic. Physician and referral information can be accessed by searching for ‘Rapid Access Clinic for Low Back Pain’ in our Find a Doctor of Service directory.
Once referred, you will be assessed by an advanced practice provider (APP) in the community within four weeks of referral. Based on this assessment, the APP will create a treatment plan.
Depending on your needs, this may include:
- education to help you manage your condition
- a personalized treatment plan
- a referral to other services or programs
If you might need surgery, we will refer you to Trillium Health Partners for further assessment and referral to a spine specialist or surgeon. We will share any assessment results and recommendations with your family doctor or nurse practitioner.

- This clinic is available in several locations throughout our community. You will be matched to a clinician close to your home.
- Appointments are 60 minutes long. Please arrive 30 minutes early for your appointment.
- Wear loose, comfortable clothing to the clinic.
- Bring a valid OHIP card and a list of your current medications.

Low back pain advanced practice providers work out of private practice clinics or family health teams across Ontario. You will be matched to the nearest provider for an appointment.
