Central West Self-Management Program (CWSMP) |
The CWSMP helps people better manage their chronic conditions and improve their health. The program offers free, non-clinical group programs. No referral is required. Online workshops are available. Participants learn:

CorHealth Ontario |
We partner with CorHealth Ontario to plan, coordinate and monitor our cardiac services. CorHealth sets standards for the quality and delivery of advanced cardiac services in the province. We meet with our CorHealth partners quarterly to review our performance and find ways to make our services even better. |
Family Medicine Teaching Centre (FMTC) |
In partnership with McMaster University's Michael G. DeGroote School of Medicine, Brampton Civic is home to the FMTC. The FMTC gives medical residents an opportunity to train with Osler family physicians and specialists and gain experience in a family medicine clinical setting. Specialty rotations include:

Princess Margaret Cancer Care Network |
The Princess Margaret Cancer Care Network is a collective of local, regional and national partners committed to facilitating collaborative clinical care, accelerating the adoption of new technologies and engaging more patients in research through improved access to clinical trials. The Network also advances innovations that improve patient outcomes and provides confidence and comfort that the best care can be achieved close to home. St. Michael's Hospital and University Health Network |
As a Regional Renal Centre, Osler works in partnership with transplant centres at St. Michael's Hospital and the University Health Network to:

Telehomecare |
We work with our local Telehomecare program to link patients at home to registered nurses at Brampton Civic and Etobicoke General. Using telephones and the internet, nurses monitor and support these patients as they manage conditions such as chronic obstructive pulmonary disease and chronic heart failure. The Telehomecare program is funded by the Ministry of Health and Long-Term Care. |
