If you are an Osler patient, you can securely access your clinical health information online through the MyChart™ patient portal. Once registered, you can use MyChart™ to review your test results and share your health information with family, caregivers, medical professionals and pharmacists. MyChart™ can help you monitor your health status and be actively involved in your care.

Sign up for an account
Provide your email address at your next Osler appointment. You will get an email with a unique PIN and secure link to the MyChart registration page. If you do not have an appointment, contact us at 905-494-2120 ext. 58864.
You will need to bring your:
- Ontario Health Insurance Plan (OHIP) card
- Government-issued photo ID (e.g. driver's license)
- Personal email address
Follow the link provided in your email or visit MyChart to complete your registration. If you have not received your PIN or have received your PIN and are having issues, contact MyChart technical support at mychart@williamoslerhs.ca or 416-494-2120 ext. 50949.
Who can use MyChart™?
All Osler patients can register for MyChart™. If a patient is younger than 14, their parent or legal guardian can contact us at 905-494-2120 ext. 56203 to access their child's records. When children turn 14, they take ownership of their own MyChart™ accounts.
What information can I access through MyChart™?
Using MyChart™, you can update your:
- Allergies: manage your allergy records
- Conditions: manage your conditions
- Diary: record your daily activities, conditions or concerns you wish to discuss with your health care providers
- Immunizations: enter and manage your immunization records
- Measurements: enter and manage measurements such as blood glucose, blood pressure, bone density, cholesterol, height, weight, etc.
- Medications: manage your medications, including prescriptions and/or any over-the-counter medication you take (e.g. pain killers, cold medications, vitamins, herbal remedies)
- Mood tracker: track your day-to-day mood and feelings
- Procedures: record any medical procedures
- Tests: upload test results from other facilities. You can request a copy of these records and have a scanned copy sent to your computer to upload to your MyChart™ account
Note: Any lab or diagnostic results and reports before April 21, 2016 will not be included in MyChart™.
You will also be able to access the following information from Osler:

View your microbiology, chemistry, core lab, blood bank, and hematology results in real time. Pathology lab results are available one day after tests are reviewed and signed. |

CT scan, mammogram, ultrasound, angiography, bone density, MRI, nuclear medicine and x-ray results are available the same day the tests are reviewed and signed by a physician. |
