The process to request information depends on the type of your request:
- To request a copy of or a correction to a patient health record:
- Visit health records or call 905-494-2120 ext. 58864
- You will be asked if you are the patient or if you are requesting a record on behalf of a patient (e.g., as a substitute decision maker, power of attorney, legal counsel, insurance company representative, estate trustee, etc.).
- To request access to non-health related records or correction to personal information:

Freedom of Information
The Freedom of Information and Protection of Privacy Act (FIPPA) provides a right of access to information held by the hospital, and outlines responsibilities with respect to privacy protection of personal information and right of access to that personal information.
Under FIPPA, we:
- provide people with access to general records about our functions and operations, such as policies and procedures;
- provide people with right of access to and correction of their personal information; and
- protect the privacy of personal information.
In keeping with the principles of FIPPA, we make our operational records publicly available whenever possible. A number of records, such as operational plans, expenses and licenses, are also available under public reporting.

What happens after I submit my request? |
When your access or correction request has been received, we will contact you to acknowledge your request. A fee estimate may also follow for search and preparation related to access requests. In accordance with FIPPA Regulation 460, fees vary depending on the resources needed to complete your request. Please note that we are not required to release records until fees have been paid, or the issue of fees has been resolved after an appeal to the Information and Privacy Commissioner of Ontario. |
When will my request be completed? |
We will process your request as soon as possible, within 30 days of receiving your complete information and issue a decision to you. If we cannot fulfill your request within 30 days we will provide you with additional notice.
