Our Reactivation Care Centre (RCC) builds on your strengths and increases your independence. We serve patients who:
- no longer need specialized acute hospital services,
- would benefit from restorative care in a quiet setting, and
- are waiting to return home or to another care facility in the community.
Your RCC care plan may include therapy, restorative care and/or activation activities. Our goal is to help you improve your ability and quality of life.
The RCC is located at:
Humber River Hospital – Church Site
200 Church Street, Etobicoke
While the building is maintained by Humber River Hospital, Osler staff care for all patients in our unit on the fifth level, B wing. Learn more about visiting the RCC including directions, transit information and parking.
What is restorative care?
The focus of restorative care is on helping you improve your quality of life and achieve your highest level of ability. At the RCC, you will receive this care in a quiet, home-like environment while you wait to transition home or to another care facility in the community.
How does the RCC support better health care?
We are committed to providing care in the right place, based on what's best for our patients. The RCC offers the best setting in which to deliver restorative care. It also allows us to better support patients who need acute hospital services by ensuring hospital beds are available for patients who need this care.
About the unit
How will this unit be different from the one I am currently on? |
At the RCC, you will continue to receive the same safe, high-quality care, but the unit will be much quieter. You won't hear overhead pages and your unit will include a common dining room and a recreation therapy space designed to best meet your needs. |
What types of patient rooms are available? |
Most RCC rooms are semi-private (two people), but some private rooms are available. Patients transferring from Brampton Civic or Etobicoke General will transfer to the same room type they requested when admitted to hospital, if available. |
What are the patient rooms like? |
Each room has been freshly painted and has its own washroom with a sink and toilet. The washrooms also feature ultraviolet technologies that help with infection control and prevention. Showers are centrally located on the unit. Rooms also have a television and telephone (local calls only), free of charge. |
Is WiFi available? |
WiFi is available in common areas on the main level. Patients and visitors can access the WiFi for free by connecting to the HRH-GUEST network. |
Are other facilities available on the unit? |
The unit has a common dining room where all meals are served. Patients unable to go to the dining room will continue to receive their meals in their room. |
Who provides meals on the unit? |
Food services are delivered by Humber River Hospital dietary aides. Patients will continue to make their breakfast, lunch and dinner selections for the week. |
About my care
Will I still be cared for by Osler staff? |
Yes. A team of professional Osler staff will continue to provide your care, seven days a week. |
Will there be a physician on the unit? |
A physician will visit you regularly. A highly skilled nurse practitioner is also available on the unit Monday to Friday. |
Will my care plan change? |
Your Osler care team will continue to follow your current care plan. Once you are settled in your new room, they will revisit the plan with you. |
What will happen if my care needs change and I need hospital services? |
We will monitor your care needs on an ongoing basis. Should your condition change and you need acute care services, we will transfer you to Brampton Civic or Etobicoke General for treatment. In the case of a medical emergency staff will call 911, just as you would if you were at home. An ambulance will take you to the closest emergency department. |
About the move
How are patients selected to be moved to the RCC? |
Our RCC unit is designed for patients who:
|
Do I have a choice to stay at Brampton Civic/Etobicoke General rather than move to the Reactivation Care Centre? Can I appeal the decision to move? |
No. The decision to transfer you to the RCC has been made by your physician and care team based on what they believe will help you achieve the best possible health outcomes. Our goal is to always ensure patients receive care in settings that are best suited to meet their ongoing needs. |
Who can I speak to if I have questions or concerns about the move? |
We want to make this move as smooth as possible for you and your family. The manager on your unit will be happy to discuss any questions or concerns you may have, or to arrange a meeting with you and your family to discuss further. If you have concerns or questions after speaking with the unit manager, you can contact our Patient Experience Office at 905-494-2120 ext. 56565 or 416-494-2120 ext. 56565. |
