During Pharmacy Appreciation Month, we recognize the extraordinary efforts of those who work behind the scenes in our pharmacies. This couldn’t have been more true throughout the pandemic while the team played in instrumental role in COVID-19 therapy management and preparing and administering vaccinations.
Together, our Pharmacy team ensures medication is prepared accurately and distributed safely at our pharmacies at Peel Memorial Centre, Etobicoke General Hospital and Brampton Civic Hospital. But they do much more than that.
Pharmacists and pharmacy technicians work together
Registered Pharmacists are medication experts who will often provide recommendations for medication use and dosing. They can adapt medication orders for patients admitted to hospital to ensure timely access to medications, prevent medication errors and ultimately reduce the number of follow-up communication needed.
Registered Pharmacy Technicians ensure patients receive the right medications through accurate preparation and dispensing. Our pharmacy technicians complete Best Possible Medication Histories to verify all of a patient’s medication use by interviewing the patient/family and reviewing at least one other reliable source. This allows for continuity of care, error reduction and informs medication treatment decisions.
Pharmacists and pharmacy technicians work closely to provide timely access to medications and medication care reviews, education and supports. While their scope in practice is different, they work collaboratively to provide safe and effective medication therapy in the hospital setting.
Pharmacists don’t just work in the Pharmacy
The Pharmacy is not just a physical space, it actually runs throughout our hospitals. Pharmacists are present on a number of different units, including our renal, seniors, oncology, diabetes, urgent and complex medical clinics and more. They provide individualized care for patients, follow up on their progression through their care journey and make recommended medication adjustments when needed.
Along with reviewing medications, making recommendations (including antibiotic use in hospital), the Pharmacy team works with doctors, nurses and other health care providers. They are an integral part of the patient care team.
Pharmacy services run 24/7
Our Pharmacy never sleeps. The team works around the clock to ensure medication is dispensed safely and accurately throughout our hospital, resulting in a continuity of care for our patients.
When pharmacies operate at a 24/7 capacity this allows for timely access to medication therapies as well as access to an on-site overnight pharmacist. Pharmacy technicians are able to distribute medications regardless of the time of day.
