About Us

Osler’s Transitional Aged Youth Clinic (TAY) is designed for youth between the ages of 16 to 24, who are struggling with mental health and live in Peel Region.

We work collaboratively with you by creating a safe space, where you have the freedom to explore opportunities to reach your personal goals and meet developmental milestones as you transition into adulthood.

Your voice matters! We respect your opinions and we want you to use your ideas, strengths and abilities to make our mental health programs fit for you. We are committed to protecting your privacy and confidentiality. We value diversity, inclusivity and youth from all ethno-cultural background, abilities, sexual and gender identities. We are here to support your journey to recovery. 


REFERRAL REQUIREMENTS:

Physician referral required.

ADDITIONAL REFERRAL DETAILS:
  • A referral should be completed by a physician.

  • The completed referral should be faxed along with any relevant psychiatric history (e.g., psychiatric reports) to the Transitional Aged Youth Clinic fax line: 905-863-2472

  • The referral will be reviewed by a centralized intake clinician, followed by a scheduled telephone intake to assure that client meets the criteria for TAY services.

  • Mood and Anxiety Group

  • Emotion Regulation Group

  • Life Skills Psychoeducation Group

  • Psychiatry 

The clinical team includes social workers, an occupational therapist, a registered nurse and consulting psychiatrists.
