Having a long-lasting chronic health condition such as diabetes can impact your physical, mental and emotional health. The good news is that you can improve your health by learning positive self-management skills.
The Central West Self Management Program
is hosted by Osler in the Central West LHIN communities and all programs are funded by the Ministry of Health and Long Term Care. Six-week Self-Management Programs support people living with many chronic health conditions, including Diabetes. Current programs offered include Chronic Disease Self-Management Program, Diabetes Self-Management Program and Chronic Pain Self –Management Program.
The programs are free. Some programs are offered in languages other than English.
The six-week program
The program runs for 2 ½ hours once a week, for 6 weeks. Topics include:

- Setting personal goals and creating realistic action plans
- Identifying and solving common problems
- Managing pain and fatigue
- Developing healthy eating and Physical activity plans
- Managing depression, stress and difficult emotions
- Medications management
- Working with health care providers
- Planning for the future
There are several program locations in Brampton, Etobicoke, Dufferin-County, Caledon, Malton, Woodbridge and Rexdale. Check here for details and dates.
