Sometimes a patient may need many different hospital services and expertise. When recovering from orthopaedic surgery or a stroke, for example, some patients require wound care, dialysis, or therapy. For patients nearing the end of their life, pain management may be of special importance. Our Complex Continuing Care and Rehabilitation teams are inspired by the very individual needs of their patients and deeply committed to giving them the best possible care.   

Complex Continuing Care

The CCC team attends to patients whose care is complex. They provide:

    • medical management and skilled nursing care
    • therapeutic services such as physiotherapy and recreation therapy
    • wound care
    • dialysis

The team’s goal is to stabilize the patient’s condition so he/she can achieve as much independence as possible.

Palliative Care

The Palliative Care team provides gentle and supportive care for patients with terminal illnesses who are nearing the end of their life. Their commitment to you:

    • Relief of your pain and other concerns identified by you will always be our first priority
    • We will encourage you to be independent as long as possible
    • We will recognize both patient and family (those identified by the patient) as our  partners in your care
    • We will discuss your concerns and needs in an open, honest manner
    • As much as we are able we will respond to your care choices and advocate on your behalf
    • We will work closely with you and your family to respect personal beliefs, values and practices
    • Whenever possible, we will help you go home

We encourage you to seek help from the palliative care team early so that they can support you and your family through difficult choices and concerns. 

The Palliative Care teams at Brampton Civic and Etobicoke General are connected to community outreach services that can provide you with additional supports. As well, three of Osler’s Palliative Care physicians follow clients in the community.

Rehabilitation

The goal of rehabilitation is to help patients regain their physical strength, mobility and independence. Osler’s rehab professionals take this commitment to heart every day, providing exemplary care to patients so that they can live life to their fullest potential.  

Lifeline – help at the push of a button

Lifeline is a Personal Response and Support Service that can give you the peace of mind to live independently at home. All it takes is the push of a button to summon help when you need it. 
Lifeline is suitable for those who are:

    • Often home along
    • At risk for falls
    • Using adaptive devices to assist with walking
    • Managing a medical condition
    • Considering a move to an assisted living facility
    • At risk for readmission to hospital
