Innovation, superb care, and strong partnerships are the hallmarks of Osler’s cardiac care program

The demand for cardiac care services in the Osler community is growing and that means we need be resourceful. While our plans include new or expanded cardiac services at Osler, we continue to seek out innovative partnerships with other hospitals and community-based services that provide any cardiac services we don’t offer. By working together, we can make the best use of precious health care resources and ensure our community has all the cardiac services it needs.

A secondary prevention program serving individuals who have chronic heart disease, heart surgeries or heart transplants.

What to expect at the CHARM clinic

Before your first visit to the clinic, you will have a two-hour education session with a dietitian, registered nurse and kinesiologist. They will talk with you about what how you can reduce your health risk. Then they’ll help you set a realistic goal for yourself and develop a plan to achieve it.

Additional visits

After this introductory session at the clinic, you may need a follow up appointment. The appointments are usually 15-45 minutes long and you might need blood work or other tests before the appointments. So please arrive at least 15 minutes before your appointment time.

Risk factors you should know about

West GTA Stroke Network (Regional Stroke Centre - Trillium Health Partners)

If you or a loved one suffer a stroke, it’s good to know you have access to the Regional Stroke Centre, located at the Trillium Health Centre, in Mississauga.

So long as the person’s type of stroke meets certain conditions, the ambulance team will take them directly to Trillium to be assessed for thrombolytic therapy (t-PA) or other possible stroke treatment. If they are well enough, patients who do not receive t-PA are transferred to the Emergency Department of their ‘home’ hospital.

24/7 Code STEMI program

Patients arriving with heart attacks arising from a blocked coronary artery bypass the Emergency Department at Brampton Civic Hospital and go straight to Osler’s cardiac catheterization lab.

At Etobicoke General Hospital, patients are immediately redirected to Brampton Civic Hospital for this care. Cardiologists use a balloon catheter and stent to quickly open the blocked artery.

Telehomecare

Osler is offering an innovative way for patients living with chronic obstructive pulmonary disease (COPD) to manage their condition at home. Osler partners with Ontario Telemedicine Network (OTN) to provide Telehomecare to patients living with COPD. Funded by the Ministry of Health and Long-Term Care, Telehomecare links patients from home to registered nurses at Etobicoke General Hospital so they can learn how to take charge of their health.

Using telephone and internet communication, nurses monitor and support people to manage conditions such COPD and Chronic Heart Failure (CHF).
