Health Information Management collects, stores and retains personal health information of the individuals who have been registered as patients within the hospital, in their Osler health record. Personal health information is a history of both past and present visits of a patient and is used by health care professionals for continuity of patient care. Health Information Management is committed to protecting patient confidentiality while providing access to personal health information to authorized persons.

How do I obtain a copy of my Osler health record?

You or your legal representative must complete a Consent to Disclose, Transmit, Access or Examine Personal Health Information.

The consent must be directed to William Osler Health System, must be signed, witnessed, dated and specify which visit and clinical information is being requested, and to whom the information is being released to.

Is there a fee for obtaining my Osler health record?

There is no fee when releasing information to physicians or other health care facilities for the purpose of continuity of care. 

For requests other than for the purpose of continuity of care, including individuals requesting a personal copy of their record, fees as outlined in a fee schedule aligned to guidelines set by the Office of the Information and Privacy Commissioner of Ontario will apply.

How do I request a copy of a record for a deceased patient?

The executor of the estate must provide proof by producing the Will and sign a Consent to Disclose, Transmit, Access or Examine Personal Health Information.

If no Will exists, a Certificate of Appointment of Estate Trustee is required.

My child was born at William Osler Health System and will be travelling out of the country with me in the near future. The official Birth Certificate has not yet arrived. How do I obtain one from the hospital?

The hospital does not issue Birth Certificates. However, a proof of birth letter will be provided upon request and completing a Consent to Disclose, Transmit, Access or Examine Personal Health Information.

A fee, as set out in the hospital's fee schedule applies.

To obtain a copy of your Birth Certificate, contact the Office of the Registrar General at 1-800-461-2156.

How do I obtain a Death Certificate?

The hospital does not issue Death Certificates. Please contact the Office of the Registrar General at 1-800-461-2156.
